  • Profit and Loss Statement
  • Balance Sheet
  • Capital expenditures - furniture and fixtures:

    Copy of invoice or list including date of purchase, total amount paid, detail description
  • List of officers including name, social security number, title, address, percentage of ownership, and compensation
  • Changes in ownership or ownership percentages
  • Percentage of income, capital gains, capital loss distributions to partners if not the same as ownership.
  • Amount of cash or property distribution to each owner
  • If profit and loss statement does not have a separate category for meals and entertainment, need this total. (Includes amount for travel, business, meetings, etc., does not include amount for travel lodging, airlines, etc.)
  • Information regarding major changes in business transactions:
    Type, Method, Payment, Etc.
