Documents Needed to Prepare 2003 Tax Return
- Completed Client Organizer
- All W-2's
- All 1099's
- All Income for dependent children or a copy of their completed tax
return
- Summary of medical expenses, including health insurance premiums,
eyeglasses, copay of prescription drugs, doctors, dentists, etc., amount
of insurance reimbursement
- Year end mortgage statement (Form 1098)
- Receipt for payment of property taxes (or cancelled check)
- Summary of charitable contributions or all receipts
- Receipts of any non-cash contributions - these must include date,
description & amount
- All K-1's received from partnerships, LLCs, S Corporations, Trusts,
Estates (Send separately if not received when everything else is ready,
but let us know you are sending it)
- Closing documents for any real esate purchases or sales
- For any stocks sol, in addition to the broker statement, we will
need the date of acquisition, the amount paid per share, dividends
earned not previously reported to us, related expenses or a statement
prepared by your broker that provides average cost information.
- For any stocks received from an estate, trust or gift, we will need
the date of acquisition and basis. (Call to discuss this)
- Any unreimbursed employee expenses. If automobile mileage we will
need total business, commuting, and total all miles driven. Driving
to work is commuting and not business miles.
- Copy of all federal or state correspondence received since our last
communication, including all return packets or mailing laels sent to
you by any taxing agencies including if you received your advance child
care credit and how much you received.
- All legal documents for formation of any type of business created
during the year
- All legal documents for divorce decrees (Child custody, property
settlement, etc.)
- Voided check or deposit slip for direct deposit of any tax refund
- If self-employed, summary of all revenue and expenses. Any assets
purchased must include date of purchase, description of asset, amount
paid
- New clients must provide a copy of their 2002 (and if possible 2001)
tax return.
